Avalon Medical Release Form
This is the Avalon Medical Release Form. This form only needs to be completed once each year and will cover your student on all of our trips/events. We will need to refresh these each fall.
Avalon Event Registration Form
This is the Avalon Event Registration Form. If you pick this up at the church, it is printed on an envelope. If you print it off from the web, please place this form, along with registration money and any additional required forms into an envelope before turning it in.
